Wishes & Values at Life's End

Presented by Seth Pollack of Compassion & Choices

End of life choices can be difficult, but ultimately valuable, conversations to have. Join your community at Silver Connections for an informative, conversation-style workshop to answer any questions you may have about planning, end-of-life rights and more. This workshop covers more than simply helping people fill out a health care proxy, MOLST form, or advance directive: it helps people think about their values and it walks them through the various planning documents they can put in place, including information on advance planning in case of dementia.

The LOFT's Silver Connections program is made possible by grants from the Westchester County Department of Senior Programs and Services, New York State Department of Health, New York State Office for Aging, and the U.S. Department of Health and Human Services.
