Wishes & Values at Life's End
Presented by Seth Pollack of Compassion & Choices

End of life choices can be a difficult, but ultimately valuable, conversation to have. Join your community at Silver Connections for an informative, conversation-style workshop to answer any questions you may have about planning, end-of-life rights, and more.
This workshop covers more than simply helping people fill out a health care proxy, MOLST form, or advance directive: it helps people think about their values and it walks them through the various planning documents they can put in place, including information on advance planning in the case of dementia.

Tuesday, October 29th | "Annual Halloween Costume Party!"

11:30am-1:00pm | Lunch provided.

Location: at The LOFT (252 Bryant Ave., White Plains)

All are welcome.
For questions, contact Shepard at shepard@loftgaycenter.org or
(914) 948-2932 x13
